Mounjaro's Efficacy in Children with Type 2 Diabetes
Eli Lilly's drug Mounjaro, containing the active ingredient tirzepatide, has shown promising results in managing blood sugar levels and weight loss among children and adolescents aged 10 to 17 with type 2 diabetes. The late-stage trial, presented at the European Association for the Study of Diabetes meeting in Vienna, involved 99 participants who received either a 5 mg or 10 mg dose of Mounjaro or a placebo weekly. The study found that Mounjaro significantly reduced A1C levels, a key measure of blood sugar control, by an average of 2.2% compared to placebo. Notably, 79% of children taking tirzepatide achieved A1C levels below 6.5%, indicating a return to non-diabetic status. Additionally, participants on the 10 mg dose experienced an 11.2% reduction in body mass index (BMI), while those on the 5 mg dose saw a 7.4% reduction. The safety profile was consistent with that observed in adults, with mild to moderate gastrointestinal side effects reported.
Orforglipron: A New Pill for Weight Loss
Eli Lilly is also advancing its experimental weight loss pill, orforglipron, which operates on the same GLP-1 receptor pathways as Mounjaro. In a separate 72-week trial involving 3,127 adults with obesity but no diabetes, participants taking orforglipron lost an average of 12% of their body weight. The study highlighted that one in five participants lost 20% or more of their body weight. The convenience of a daily pill could potentially increase access to obesity treatment compared to injectable options like Mounjaro and Wegovy. The trial demonstrated improvements in cardiovascular risk factors, including cholesterol and blood pressure, although the weight loss results were less dramatic than those achieved with Mounjaro.
Official Statements & Responses
Kenneth Custer, president of Lilly's cardiometabolic health division, emphasized the significance of Mounjaro's results in children, stating, "This is about taking a meaningful medicine that's made a great difference for adults, and showing that it has similar encouraging results in kids." Regarding orforglipron, Custer noted that it represents "the most promising approach to creating an effective small-molecule GLP-1."
Criticism & Opposition
Despite the positive outcomes, some experts caution that the long-term effects of GLP-1 receptor agonists in children and adolescents require further investigation. Concerns have been raised about the potential for gastrointestinal side effects, which were noted in both trials. Additionally, while orforglipron's convenience as a pill is highlighted, experts stress the need for comprehensive studies to confirm its efficacy and safety compared to existing treatments.
Conflicting Reports & Gaps
While the trials for both Mounjaro and orforglipron indicate significant benefits, discrepancies exist regarding the robustness of weight loss results. The orforglipron trial reported a plateau in weight loss after 72 weeks, contrasting with earlier studies that suggested more sustained reductions. Further detailed results from these studies are anticipated at future medical meetings.
What's Next
Eli Lilly has submitted the new data on Mounjaro for regulatory review to support its expanded use in children. The company also plans to present detailed findings from the orforglipron trial at upcoming medical conferences, as it seeks to establish its position in the competitive landscape of diabetes and obesity treatments.
