New Genetic Tool for Dementia Prediction
Recent research led by Professors Kim Hee-jin and Won Hong-hee from Samsung Medical Center, alongside Professor Seo Jin-soo from Yonsei University, has introduced an innovative tool called the optimized polygenic risk score (optPRS). This tool aims to enhance the prediction of Alzheimer's disease risk by analyzing genetic variations associated with the condition. The optPRS, developed using genetic and health data from over 1,600 Korean patients, demonstrated that individuals with higher scores had approximately double the risk of developing Alzheimer's and amyloid protein accumulation in the brain. Understanding Dementia and Its Modifiable Risk Factors

Richard Oakley, associate director of research and innovation at the Alzheimer's Society, emphasizes that while aging is a major risk factor, there are 14 modifiable factors that can significantly reduce the risk of developing dementia. These include lifestyle changes such as maintaining a healthy weight, engaging in regular exercise, and avoiding smoking and excessive alcohol consumption. The Lancet Commission has compiled these factors, highlighting that nearly 45% of dementia cases could be preventable through proactive health measures. Companies like Alector and GSK are developing treatments aimed at increasing progranulin levels, a protein linked to FTD. Notably, the drug AL001 has received multiple designations from the US FDA, including orphan drug and breakthrough therapy status, indicating its potential significance in treating this condition. Other promising therapies in development include AVB-101, a gene therapy designed to restore normal progranulin levels through targeted delivery to the brain.
Criticism and Opposition
Despite the advancements in genetic testing and lifestyle interventions, some experts caution against over-reliance on genetic predispositions. Critics argue that while genetic tools like optPRS can identify high-risk individuals, they do not account for the multifactorial nature of dementia, which includes environmental and lifestyle factors. Additionally, there is concern that focusing too heavily on genetic risk may lead to fatalism among individuals who perceive themselves as genetically predisposed to dementia.
