Recent clinical trials in Africa and Europe have indicated that a "functional cure" for HIV may be achievable, allowing some patients to maintain viral suppression without continuous antiretroviral therapy. Approximately 40 million people globally are living with HIV, which traditionally requires lifelong treatment. However, two independent trials—the FRESH trial in South Africa and the RIO trial in the United Kingdom and Denmark—have shown that engineered antibodies can help control the virus long-term.
In the FRESH trial, led by virologist Thumbi Ndung’u, 4 out of 20 participants maintained undetectable HIV levels for a median of 1.5 years after receiving antibody infusions. Similarly, in the RIO trial, 6 out of 34 participants sustained viral control for at least two years. These findings suggest that the immune system can be effectively harnessed to combat HIV.
Mechanisms Behind the Trials
Participants in both trials received modified antibodies designed to last approximately six months. Following the infusion, their antiretroviral treatments were paused to assess the antibodies' effectiveness in controlling HIV. Notably, in the RIO trial, 22 participants did not experience a viral rebound within 20 weeks, and six maintained low viral levels beyond 96 weeks, allowing them to remain off antiretroviral medications. The FRESH trial also demonstrated that some participants retained viral suppression for over a year after the antibody infusion.
Researchers believe that these antibodies may stimulate an immune response akin to vaccination, potentially enhancing the activity of CD8+ T cells, which target HIV-infected cells. This immune memory could help control the virus even after the antibodies are no longer present.
Challenges and Future Directions
Despite the promising results, achieving a complete cure remains complex due to HIV's ability to hide in dormant reservoirs within the body. The RIO trial showed that the antibodies could impact these dormant cells, while the FRESH trial incorporated vesatolimod, a drug aimed at stimulating immune responses to flush out hidden HIV.
Experts like Joel Blankson from Johns Hopkins Medicine emphasize the importance of early antiretroviral treatment initiation for better outcomes. Future studies aim to explore whether delaying antiretroviral treatment before administering antibodies can enhance immune responses.
