Fatal Incident on the M5

On November 30, 2023, an 18-year-old man tragically died after exiting an ambulance on the M5 motorway in North Somerset. The ambulance had pulled over between junctions 21 (Weston-super-Mare) and 20 (Clevedon) to provide further treatment while en route to the hospital. At approximately 11:04 PM, police were alerted that the patient had left the ambulance and was on the motorway. An officer attempted to guide the young man to safety; however, he was struck by a southbound vehicle shortly thereafter and pronounced dead at the scene.

The M5 was closed for nearly ten hours to facilitate emergency response and investigation efforts. Assistant Chief Constable Joanne Hall expressed condolences to the victim's family and noted the emotional impact on the officers and ambulance crews involved. A mandatory referral to the Independent Office for Police Conduct (IOPC) has been made to investigate the circumstances surrounding the incident.

Minibus Collision in North Yorkshire

In a separate incident on December 1, 2023, a 16-year-old girl named Eleanor Hague died following a collision involving a minibus and a Hyundai i10 in Tadcaster, North Yorkshire. The crash occurred around 12:28 PM on Toulston Lane, near Tadcaster Grammar School, while the minibus was carrying nine passengers aged 16 to 18. Another teenage girl sustained serious but non-life-threatening injuries, while the remaining passengers and the minibus driver suffered minor injuries.

The driver of the Hyundai, a 40-year-old woman, has been arrested in connection with the investigation. North Yorkshire Police are appealing for witnesses and have urged anyone with dashcam footage to come forward. The school announced its closure for safety reasons following the incident, and local community leaders expressed their condolences to the families affected.

Official Statements & Responses

Assistant Chief Constable Joanne Hall stated regarding the M5 incident, "Our thoughts are with the man's family at such a devastating time. We will support them in any way we can." She also highlighted the emotional toll on emergency personnel involved in the incident. Similarly, Tadcaster Grammar School expressed deep sadness over Eleanor Hague's death, stating, "Our thoughts are with everyone affected by this unimaginable tragedy."

While the investigations into both incidents are ongoing, there has been public concern regarding the safety measures in place for patients being transported by ambulance and the protocols followed during emergencies. Community leaders in Tadcaster have called for a thorough investigation to ensure accountability and prevent future tragedies.

Conflicting Reports & Gaps

Both incidents have raised questions about the circumstances leading to the fatalities. In the M5 case, the exact reasons for the teenager exiting the ambulance remain unclear. In Tadcaster, the details surrounding the minibus collision and the actions of the Hyundai driver are still under investigation.
