Current treatment options are limited, with liver transplantation being the only definitive solution, but only a small fraction of patients qualify for this procedure.

Mechanism of the Dialive Device

Dialive operates similarly to kidney dialysis, removing dysfunctional albumin from the blood and replacing it with functional albumin. This process aims to alleviate the toxic burden on the liver, allowing it to regenerate and potentially reversing the condition. Rajiv Jalan, a senior liver specialist and co-principal investigator of the trial, emphasized the urgent need for effective treatments for ACLF, stating, “These are gravely ill patients with multi-organ failure and high risk of death.”

Funding and Previous Research

The trial, funded by the National Institute for Health and Care Research (NIHR) at a cost of £2.2 million, follows promising results from a smaller study conducted in 2023. In that study, 10 out of 15 patients treated with Dialive showed reversal of ACLF, compared to only five in the control group. Dr. Rohit Saha, another co-principal investigator, noted that Dialive could potentially break the cycle of inflammation that often leads to patient mortality.

Implications for Patient Care

If successful, Dialive could significantly alter the treatment landscape for ACLF, reducing the need for liver transplants and improving patient outcomes. Prof. Mike Lewis, NIHR's scientific director for innovation, remarked on the potential life-saving benefits of the device, stating, “If this NIHR-funded trial of Dialive is successful, it will bring dramatic benefits for patients, providing a treatment for ACLF that will save lives and reduce hospital stay.”

Criticism and Concerns

While the trial represents a hopeful advancement in liver disease treatment, some experts caution about the broader implications of relying on new technologies in healthcare. Concerns have been raised regarding the accessibility of such treatments and the potential for disparities in patient care.

The trial is scheduled to commence early next year, with hospitals including King’s College and University College London leading the recruitment of participants.
