The study analyzed 859 cases of traveler’s diarrhea treated at 58 tropical medicine centers worldwide between April 2015 and December 2022.

The findings indicate significant geographic variations in antibiotic resistance patterns. For instance, approximately 79% of Shigella cases in South Central Asia were resistant to fluoroquinolones, while 78% of cases in South America showed resistance to macrolides. This suggests that the effectiveness of antibiotics can vary greatly depending on the region where the infection is contracted.

Causes of Increasing Resistance

Experts attribute the rise in antibiotic resistance primarily to the overuse of these medications. Dr. David Purow, vice president of medicine and GI endoscopy at Northwell Health, emphasized that bacteria evolve over time, leading to increased resistance. He noted, “Bacteria are smart and they can evolve over time. That’s how resistance develops.” This evolution is exacerbated by the indiscriminate use of antibiotics by travelers who often self-medicate without knowing the specific bacteria causing their illness.

Recommendations for Travelers

Both the researchers and Dr. Purow recommend that travelers experiencing diarrhea consult a healthcare professional for appropriate testing to determine the most effective antibiotic for their specific condition. Many travelers currently carry antibiotics and take them at the first sign of symptoms, which may not be necessary. Dr. Purow suggests that over-the-counter medications can alleviate symptoms without the need for antibiotics, as many gastrointestinal infections resolve on their own.

He cautioned against the premature use of antibiotics, stating, “Just taking antibiotics without knowing specifically what they’re treating can have some ramifications for the global population.” Delaying antibiotic use until symptoms worsen could help mitigate the growing issue of antibiotic resistance.
