Eli Lilly Targets 2027 Approval for Next-Generation Obesity Drug Retatrutide

Eli Lilly announced that it will submit a regulatory application for retatrutide in the first quarter of 2027. The company said additional time is required to gather and verify manufacturing and quality-control data before seeking approval. Kenneth Custer, president of Lilly Cardiometabolic Health, stated that the data from recent late-stage trials give the company confidence to file globally for obesity, knee-osteoarthritis pain, and obstructive sleep apnea.

Trial Outcomes and Safety Profile

Two Phase 3 trials demonstrated substantial weight loss in adults with obesity and major health complications. In the diabetes cohort, participants lost an average of 20.8 % of body weight—about 50 pounds—over 80 weeks. In a separate study of severe obesity with established cardiovascular disease, average loss reached 22.6 % (approximately 55.8 pounds) at the same time point. Both trials also showed meaningful reductions in key cardiovascular risk factors and improvements in blood-sugar measures. Reported adverse events were consistent with the broader GLP-1 drug class, the most common being diarrhea, nausea and constipation.

Mechanism and Portfolio Positioning

Retatrutide is dubbed the “triple G” drug because it simultaneously targets glucagon-like peptide-1 (GLP-1), glucose-dependent insulinotropic polypeptide (GIP) and glucagon, unlike existing treatments that engage one or two of these hormones. By contrast, tirzepatide (the active ingredient in Lilly’s Zepbound) mimics GLP-1 and GIP, while Novo Nordisk’s semaglutide (Wegovy) mimics only GLP-1. Lilly positions retatrutide as the next pillar of its obesity portfolio, following the weekly injection Zepbound and the newly launched oral pill Foundayo.

Market Projections and Competitive Stakes

Analysts at TD Cowen estimate retatrutide could generate roughly $3.8 billion in sales by 2030. The drug is viewed as critical to preserving Lilly’s market-share advantage over Novo Nordisk in the rapidly expanding weight-loss and diabetes segment, which some forecasts value at about $100 billion by the 2030s.
