Severe COVID-19 Hospitalizations Linked to Reactivation of Dormant Viruses

A study of more than 1,000 patients hospitalized with COVID-19 in 2020 and early 2021 found that nearly half experienced reactivation of latent viruses that normally remain dormant in the body. The reactivated pathogens included herpesviruses such as Epstein–Barr virus (EBV) and cytomegalovirus (CMV), as well as generally harmless anelloviruses. Reactivation was observed across a range of immune statuses, including individuals without known immunodeficiency.

Study Design and Quantitative Results

Researchers collected blood, nasal swabs, and lung-fluid samples from the participants and screened for viral DNA. The analysis showed:

  • Reactivation prevalence: Approximately 45 % of the cohort showed evidence of at least one dormant virus becoming active.
  • Timing of reactivation: EBV and anelloviruses peaked early, shortly after hospital admission, while CMV and herpes simplex virus (HSV) tended to emerge about three weeks later, primarily in the respiratory tract.
  • Immune-system impact: The likelihood of viral awakening was not higher among patients receiving immunosuppressive drugs compared with those who were not, indicating that the stress of SARS-CoV-2 infection alone can overwhelm immune control.

Biological Interpretation

Esther Melamed, a physician-scientist in neuroimmunology at the University of Texas at Austin and co-author of the analysis, explained that severe COVID-19 likely induces physiological stress that triggers viral reactivation, which then amplifies inflammation. Resia Pretorius, a physiologist at Stellenbosch University in South Africa, noted that the findings overturn the prior assumption that such reactivations occur only in immunocompromised individuals, such as people living with HIV.

Potential Clinical Implications

If subsequent research confirms that reactivated viruses contribute to the severity of acute illness or the development of long COVID, the results could open new therapeutic avenues. Targeting specific viral reactivations or mitigating the stress response may become part of comprehensive COVID-19 treatment strategies, especially for patients experiencing prolonged symptoms after the initial infection.
