Arthritis Drug Upadacitinib Shows Strong Hair Regrowth in Severe Alopecia Areata Trials

Researchers at Harvard University conducted two large, international clinical trials to evaluate upadacitinib—an oral Janus kinase (JAK) inhibitor originally approved for arthritis—in patients with severe alopecia areata. The studies enrolled participants at medical centers across North America, Europe and China. Participants received either a 15 mg dose, a 30 mg dose, or a placebo once daily for 24 weeks. The results were published in *JAMA Dermatology* on August 12 and demonstrated a marked difference between the drug and placebo groups.

Participant Characteristics

All enrolled individuals had extensive hair loss, with an average scalp-hair loss of about 84 percent at baseline. Many had experienced no meaningful regrowth for at least six months before entering the trials, underscoring the chronic and treatment-resistant nature of their condition.

Efficacy Findings

Across both dosage arms, roughly half of the participants achieved at least 80 percent scalp-hair coverage after 24 weeks. A subset of patients experienced complete or near-complete restoration of scalp hair. By contrast, participants receiving placebo showed substantially lower rates of regrowth. The magnitude of improvement was noted despite the participants beginning the studies with very little remaining hair.

Potential Impact on Alopecia Areata Treatment

Alopecia areata is an autoimmune disease in which the immune system attacks hair follicles, leading to unpredictable hair loss that can affect the scalp, eyebrows and body. Upadacitinib’s mechanism—blocking inflammatory signals that drive the autoimmune response—offers a targeted approach distinct from existing therapies, which have limited efficacy in severe cases. If further studies confirm these findings, upadacitinib could become a valuable option for patients who have exhausted other treatments, potentially reducing the psychological burden associated with extensive hair loss.
