Hormone-Replacement Therapy Shortages Prompt Emergency Supply Protocol in Northern Ireland

By Drooid · · How we work

Background and Scope of the Shortage

Supply chain disruptions have affected several hormone-replacement therapy (HRT) products across the United Kingdom. The Department of Health identified ongoing shortages of Estradot transdermal patches and a more recent interruption affecting FemSeven Conti patches. These disruptions have prompted the activation of a Serious Shortage Protocol (SSP) for Estradot patches, which remains in effect until 8 January 2027.

Official Response and Guidance

The Department of Health confirmed that communications have been sent to healthcare professionals outlining the products impacted and recommending suitable alternative formulations that remain available. Under the SSP, pharmacists are authorised to dispense alternative HRT products without requiring a new prescription, provided the patient consents. The department advises patients who cannot obtain their prescribed medication or who experience concerns with an alternative to consult their GP for further advice.

Impact on Patients

Ursula Mason, chair of the Royal College of General Practitioners Northern Ireland, highlighted that substituting a familiar HRT preparation can cause significant anxiety, especially when a woman has found a formulation that effectively manages her symptoms. She noted that the protocol permits only substitutions with the same active ingredient at the same dose, aiming to minimise clinical differences. Mason urged patients experiencing side effects or inadequate symptom control with a new formulation to discuss options with their GP.

Outlook and Ongoing Measures

The Department of Health’s ongoing communications aim to keep clinicians informed about available alternatives and to monitor the shortage’s progression. While the SSP provides a temporary mechanism to maintain treatment continuity, the underlying supply issues remain unresolved, and the department has not specified a timeline for restoring normal availability of the affected HRT products.
