Personalized mRNA Melanoma Vaccine Cuts Recurrence Risk by 42% in Phase-3 Trial
By Drooid · · How we work
Trial Results and Key Findings
A late-stage, randomized study of 1,137 adults with fully resected stage IIB-IV melanoma compared Merck’s pembrolizumab (Keytruda) alone with pembrolizumab plus a patient-specific mRNA vaccine, intismeran autogene. At a median follow-up of 24.6 months, the combination reduced the relative risk of recurrence or death by 42% and the risk of distant metastasis or death by 38% versus pembrolizumab alone. Two-year recurrence-free rates were 82.2% with the vaccine and 70.0% with pembrolizumab alone. No new safety signals emerged; grade 3-5 treatment-related adverse events occurred in 14.2% of vaccine recipients versus 13.4% of controls. Overall survival data remain immature.
Background and Development
The vaccine builds on mRNA technology first deployed for COVID-19. For each patient, tumor DNA and RNA are sequenced, neoantigen-producing mutations are identified with machine-learning models, and up to ~34 peptide targets are encoded into a bespoke mRNA construct delivered in lipid nanoparticles. The approach was pioneered in a phase-2b trial that showed a 49% reduction in recurrence risk, prompting the larger phase-3 effort.
Official Statements & Responses
Merck’s chief medical officer Eliav Barr said the data represent an early step and that the company will seek FDA approval, potentially as early as next year.
Market Reaction and Investor Outlook
Following a partial leak of the data on October 22, Moderna’s shares fell up to 26% intraday and closed the next day down 17.57% at $202.63. By October 23 the stock was $200.10, down 1.25% after a sharp selloff. Shares remain more than three times their pre-announcement level. Analysts at RBC, UBS and TD Cowen described the results as clinically meaningful but noted the benefit fell short of the larger reduction seen in the phase-2b study.
Manufacturing and Access Challenges
The individualized nature of intismeran autogene means each dose requires a separate manufacturing run. Production, including tumor sequencing, neoantigen selection and mRNA synthesis, takes roughly six weeks. While the trial achieved a 99% success rate in vaccine creation, scaling this process outside controlled study conditions remains uncertain. Cost considerations are prominent; Merck declined to estimate pricing, but the bespoke workflow suggests a premium product.

What’s Next
Merck and Moderna plan to file for FDA approval, targeting a 2027 decision. Ongoing trials are evaluating the same personalized mRNA platform in kidney, bladder and non-small-cell lung cancers. The next major data point will be mature overall-survival results from the melanoma study, which will inform both regulatory review and commercial projections.
