Pupil Fluctuations Reveal Subtle Signature of ADHD in Children

Study Overview

Researchers at the Indraprastha Institute of Information Technology Delhi reexamined eye-tracking recordings from 50 Chilean children aged 10-12, including 28 with combined-type ADHD and 22 without neurodevelopmental diagnoses. While participants performed a memory-game task, an eye tracker captured pupil diameter at 1,000 Hz across 160 trials per child. The analysis focused on trial-to-trial and within-trial variability of pupil size rather than average diameter.

Key Findings and Statistics

After adjusting for age, IQ and task difficulty, the statistical model indicated that unmedicated children with ADHD showed roughly 23 % greater trial-to-trial variability in pupil responses compared with controls. Within each trial, however, variability was lower for the ADHD group. Greater trial-to-trial variability correlated with lower accuracy on the memory task, although the strength of this relationship did not differ significantly from that observed in the control group. In a subset of 17 ADHD participants who completed the task both on medication (methylphenidate) and after a 24-hour washout, medicated trials displayed stronger pupil responses, reduced between-trial fluctuations, and response patterns more closely resembling those of the control children.

Official Interpretation

Limitations and Future Directions

The authors cautioned that pupil size is influenced by multiple physiological factors, making it an indirect marker of attention. The medication comparison was spaced by an average of six months, introducing possible maturation or practice effects. Moreover, statistical significance was narrowly missed after correction for multiple tests. The researchers called for larger, independent studies in real-world environments before pupil measurements could be considered a reliable diagnostic adjunct.

Potential Clinical Role

If validated, eye-tracking could complement existing diagnostic procedures—medical assessments, personal history, and behavioral observations—by providing a dynamic view of how an individual’s attention shifts over time or responds to treatment. At present, the technique is not positioned to replace traditional evaluation methods.
